Instructions to Submit Non-Exam Qualification Requirements

  • All requests for qualification must be sent via fax (813-864-2595) to our office.
  • All requests must be accompanied by a CTQP Qualification Application , which can be found on the CTQP homepage, printed, completed and faxed to CTQP at the number above.
  • Any request for a qualification which does not require a CTQP exam (i.e. Aggregate Field, Grouting, Post-Tensioning, etc.) must also be accompanied by a Trainee Personal Data Sheet, which can be found on the CTQP homepage.
  • Please include a contact phone number on the qualification application.
  • Please allow fourteen days for your request to be processed.
  • If there are any questions, the CTQP staff is available by phone at 813-864-2595, Monday through Friday, 8 am to 5 pm.

Instructions to submit an American Concrete Institute (ACI) certificate

Please include the following:
  • ACI identification number which can be found on the trainee's pocket card or on most ACI correspondence (there is a space to include this on the qualification application).
  • Completed CTQP Qualification Application
  • Fax all the above items to CTQP at 813-864-2595

Please note that any qualification that requires an ACI certificate, the expiration date of the qualification shall be the expiration date of the ACI or five (5) years, whichever is earlier.

Once the qualification expires, the ACI must be renewed and the new certificate faxed to us. If a CTQP exam is required for the qualification (i.e. FDOT Concrete Field Specification Technician Exam, Concrete Lab Exam, etc.), it must always be re-taken before the qualification may be updated, even if the previous qualification was less than 5 years.

CTQP suggests that the ACI certification and the CTQP exams be done within the same year to gain the maximum qualification time and avoid any confusion in this matter.

Aggregate Field Exams are given solely through ACI. The Center for Transportation Training at the University of Florida administers these exams for FDOT personnel only.  Non-FDOT personnel must go through ACI for their Aggregate Field exams.

While the Center for Transportation Training does administer these exams, they are still graded by ACI. Currently, ACI takes approximately 2-4 weeks to grade and send certificates to test-takers. Grades are not uploaded to the CTQP database, nor are they graded by CTQP. When the trainee receives the ACI certificate, please refer above for instructions on how to submit it to CTQP so that a qualification may be granted.

To submit a Nuclear Safety Certificate:

Please include the following:

  • Nuclear Gauge Safety Certificate
  • Completed CTQP Qualification Application
  • Fax all the above items to CTQP at 813-864-2595

Nuclear safety certificates are no longer required to be updated.  Once the initial certificate is received and registered with CTQP, it does not need to be renewed.

All Nuclear Gauge Certificates are accepted, provided they are accompanied by a signed Qualification Application. HAZMAT Certificates are not accepted.

To Submit a Grouting Certificate:

  • Grouting certificate
  • Printout from ASBI Web site showing which class the trainee participated in (call 813-864-2595 for details if you don't know how to do this)
  • Completed qualification application
  • Trainee personal data sheet, if the trainee does not have any other CTQP qualifications
  • Fax all of the above to CTQP at 813-864-2595

Grouting qualifications expire on the dated noted on the certificate.

  To Submit a Post-Tensioning Certificate:

  • PTI certificate
  • Copy of PTI card (please enlarge on a photocopier first)
  • Completed qualification application
  • Fax all of the above to CTQP at 813-864-2595

Post-tensioning certificates expire after four (4) years.  Please also note the listed experience on the certificate: if no experience is listed, it is a Level I certificate; if 2000 hours of experience is listed, it is a Level II certificate.


To Submit Experience for Any Qualification Area:

  • Fill out a CTQP Qualification Application complete with the signature. Check the 'Experience' box and specify the area in which you would like the experience added.
  • Fax the above to CTQP at 813-864-2595

Class Exit Survey Summary Reports

  • Published quarterly in the "Provider Login"
  • Providers can see how the Trainees felt about the CTQP Class and Testing experiences


Past Announcements

Notice: On February 24, 2006 the Florida Department of Transportation's State Construction Training Administrator requested that the CTQP Administrator Advise all providers as follows: The Re-Qualification requirements for Asphalt Plant Level 1 and Asphalt Paving Level 1 are changing. The proficiency exams formerly required only for the initial qualifications in these areas will also be required for requalification on or after May 1, 2006. This notice supersedes all previous notices showing an earlier effective date for this requirement.

Effective May 1, 2006 requirements for Requalification in Asphalt Plant Level 1 are as follows

(1) Pass the Asphalt Plant Level I written examination.

(2) Pass the Asphalt Plant Level I proficiency examination.

Effective May 1, 2006 requirements for Requalification in Asphalt Paving Level 1 are as follows

(1) Pass the Asphalt Paving Level I written examination.

(2) Pass the Asphalt Paving Level I proficiency examination.
